If you are requesting a distribution or a loan from your qualified plan please complete the applicable form below and email payout@HCSRetirement.com or fax 801.224.1930.

• Distribution Form
• Loan Application
• Hardship Distribution

Your plan may or may not allow for loans and/or hardship distributions. Please check with your plan administrator. Also note that if your plan allows for hardship distributions, you must exhaust all loan provisions first.

Additional forms may be required. We will contact you upon receipt of the completed forms or you may contact us at payout@HCSRetirement.com or phone 801.224.1900 to request the forms specific to your plan.
